Andrea Sheehy, Chief Executive Officer

With more than 16 years of experience in healthcare operations and clinical leadership, Andrea Sheehy brings a deep understanding of rehabilitation services and a passion for patient-centered care to her role as Chief Executive Officer of Rehabilitation Hospital of Northwest Ohio.

Andrea’s career has spanned acute rehabilitation, long-term acute care, and outpatient services, where she has been instrumental in advancing clinical excellence, team development, and innovative therapy practices. Prior to her appointment as CEO, Andrea served as the hospital’s Director of Therapy, where she played a key role in shaping both operational and clinical strategies to enhance patient outcomes.

Andrea earned both her bachelor’s and master’s degrees in communication disorders from Bowling Green State University in Bowling Green, Ohio. She is certified to conduct fiber optic endoscopic evaluations of swallowing (FEES).

An active member of the American Speech-Language-Hearing Association, Andrea continues to demonstrate a strong commitment to excellence, innovation, and community engagement in rehabilitation care.

Mallory Horstman RN, BSN, MPH, Director of Nursing

Mallory Horstman, Chief Nursing Officer at the Rehabilitation Hospital of Northwest Ohio, brings more than 16 years of nursing experience and a deep commitment to clinical excellence, patient safety, and quality outcomes.

A proud alumna of the University of Toledo, Mallory earned both her Bachelor of Science in Nursing and Master’s in Public Health. She is passionate about empowering nurses to lead with confidence and purpose. Mallory finds great fulfillment in the field of rehabilitation nursing—where every day offers the opportunity to support patients on their journey to recovery and renewed independence.

Mallory believes that exceptional patient outcomes are the result of strong teamwork, compassionate care, and a culture rooted in accountability and continuous improvement. She is dedicated to fostering a positive and collaborative environment that supports professional growth, enhances patient experiences, and ensures the highest standards of care.

Dr. Steven Farrell, Medical Director

Dr. Farrell has been serving the residents of northwest Ohio and southeastern Michigan for over two decades. He is dedicated to providing the best outcomes for those with functional disabilities to include neurologic and musculoskeletal injuries. He is board-certified in Physical Medicine and Rehabilitation.

Dr. Farrell earned a bachelor’s degree in biology from The University of Toledo and completed medical school and residency training at the Medical College of Ohio (now known as the University of Toledo Medical Center) in Toledo, Ohio. Fellowship training was obtained at Buffalo Spine and Sports Medicine in Buffalo, New York.

Dr. Farrell has been on the faculty of The University of Toledo College of Medicine for over 25 years. He is a professor and Division Chief of Physical Medicine and Rehabilitation. Other roles held include residency program director and clerkship director. He is a member of the American Academy of Physical Medicine and Rehabilitation, the Academic Association of Physiatry, and the Ohio Society of PM&R, where he holds the role of treasurer.

Shawn Gerken, MOT, Director of Marketing and Business Development

As Director of Marketing and Business Development at the Rehabilitation Hospital of Northwest Ohio, Shawn Gerken is passionate about connecting patients, families, and healthcare professionals with the hospital’s specialized rehabilitation services. She leads initiatives to strengthen community partnerships and raise awareness of the hospital’s programs.

With more than 12 years of experience as an Occupational Therapist—serving in the Veterans Affairs (VA) system and at a Level I trauma center in Chicago—Shawn brings a deep understanding of patient care to her role. Her clinical background ensures that every outreach effort reflects the hospital’s mission to restore independence and improve quality of life.

A graduate of the University of Toledo with a bachelor’s degree in Exercise Science and a master’s degree in Occupational Therapy from the University of Findlay, Shawn is dedicated to helping individuals rebuild their lives after injury or illness through compassionate communication, collaboration, and community education.

Monika Pawar, DPT, Director of Therapy

Monika brings more than 16 years of experience as a physical therapist to her role in leading the hospital’s therapy services. She has been an integral part of the rehabilitation team since the RHNO’s opening, serving as Lead Physical Therapist for nearly a decade before stepping into a Director of Therapy role in 2023.

Monika earned her Doctor of Physical Therapy degree from Northwestern University and has extensive experience in acute rehabilitation and interdisciplinary care. She is passionate about advancing patient outcomes through evidence-based practice, staff development, and collaboration across therapy disciplines.

Monika is dedicated to fostering an environment of clinical excellence and compassionate care for every patient and team member she serves.

Amanda Wenzinger, Sr. Director of Human Resources

Amanda brings 30 years of Human Resources Management experience to the hospital, with an emphasis on employee and labor relations.

With the majority of her career having been spent in hospital leadership, Amanda works daily to foster an environment where all staff see the importance of their role and the impact that they each have on patients and on the team.
Amanda believes in treating our employees and patients with dignity and respect and focuses on being approachable and an employee advocate.

Amanda provides direction and leadership in recruitment, retention, employee relations, and employee engagement for the hospital. She maintains and ensures Joint Commission standards are met, federal and state laws are followed, and hospital and Ernest Health policies are practiced.

Amanda earned a bachelor’s degree from Miami University in Oxford, Ohio. Amanda is a Toledo native, and has been with our hospital since it’s inception.

Ross Kwiatkowski, PharmD, Director of Pharmacy

As Director of Pharmacy at the Rehabilitation Hospital of Northwest Ohio, Ross Kwiatkowski oversees all aspects of the hospital’s pharmacy operations, ensuring the safe, effective, and timely delivery of medications to support optimal patient outcomes. He is committed to maintaining the highest standards of quality, accuracy, and regulatory compliance while advancing the hospital’s mission of providing exceptional rehabilitative care.

Ross brings more than 16 years of pharmacy experience to his role, including an extensive background in retail pharmacy prior to joining RHNO. His expertise in medication management, patient education, and interdisciplinary collaboration plays a vital role in supporting the recovery and well-being of patients throughout their rehabilitation journey.

A graduate of the University of Toledo, Ross earned his Doctor of Pharmacy (PharmD) degree and continues to give back to his alma mater by serving as a preceptor for University of Toledo IPPE and APPE students. He is passionate about mentoring future pharmacists and fostering a culture of teamwork, clinical excellence, and continuous learning within the hospital.

Sarah Poffenbaugh, LSW, Director of Patient Outcomes

Sarah Poffenbaugh serves as the Director of Patient Outcomes where she leads initiatives focused on enhancing the quality, safety, and effectiveness of patient care. A licensed social worker with over a decade of experience across the healthcare continuum, Sarah brings a unique blend of clinical insight and operational expertise to her role.

Before joining Ernest Health’s leadership team, Sarah worked as a Payor Relations Specialist for the organization, specializing in Medicare Advantage and developing strategies to improve access, authorizations, and reimbursement processes. Her prior experience includes serving as a Clinical Quality Program Consultant where she advanced quality improvement initiatives and data-driven care strategies, as well as working as a Social Worker and Care Navigator in the acute care setting, supporting complex discharges and patient-centered care coordination.

Sarah earned her Bachelor of Science in Social Work with a minor in Gerontology from Bowling Green State University in 2010. She is passionate about empowering patients to achieve their highest level of independence and recovery while fostering collaboration among care teams to drive exceptional outcomes.

Lesley Wood, RN Director of Compliance

As Director of Compliance at the Rehabilitation Hospital of Northwest Ohio, Lesley Wood plays a vital role in ensuring that every patient receives safe, high-quality, and compassionate care. She oversees all aspects of quality, regulatory compliance, and patient safety, working closely with clinical and operational teams to uphold the highest standards of performance and accountability.

With more than 35 years of nursing experience, Lesley brings a deep and diverse clinical background to her role. Her career spans acute hospital care, ambulatory care, and two decades dedicated specifically to patient safety, infection prevention, quality improvement, and regulatory oversight. Her expertise enables her to guide teams through complex healthcare regulations while fostering a culture of continuous improvement.

Lesley earned her associates nursing degree from Monroe County Community College and remains committed to advancing best practices that support exceptional patient outcomes. She is passionate about equipping staff with the tools, education, and support they need to deliver care that is both safe and consistently aligned with organizational and regulatory standards.
